New Client/Patient Form - If you are bringing in a pet that we have never seen before, even if you are already a client. This can be printed and filled out by hand, or typed in and printed out, or typed in, saved as "PetnameLastname.pdf" and emailed back to us at info@brandonlakesanimalhospital.com/a>.

Records Release - If you are boarding your pet and need proof of vaccinations.

Records Request - If you are bringing your pet to us from another veterinarian, please send them a faxed copy of this form so they will send us a copy of your pets medical history.

Care Credit - Care Credit is a type of credit card that can only be used in select medical, dental, and veterinary offices. We highly recommend applying because you never know when an emergency may strike your pet.
